Once you have scheduled a new patient chiropractic visit, the next step is to fill out a new patient paperwork packet. When you first arrive at Radiant Life, we will have the paperwork ready for you to fill out. If you prefer to fill out the paperwork beforehand, please download and print the appropriate PDF file below to access the new patient paperwork packet.

In order to assist with the cost of care, we have partnered with ChiroHealth USA to offer a discount option for cash-paying patients or for patients whose insurance we do not accept.
